Call (813) 254-5200 or book online. Your first visit is our $79 New Patient Special — no insurance, no runaround.

Fill out the intake form below (men's or women's) before you arrive. It helps us understand your full history so we make the most of your time.

Your first visit

A full 45–60 minutes: a complete exam, a review of your health history, a one-on-one consultation with Dr. Parker, and time to talk through what's really going on.

Your personalized roadmap

We identify the right functional testing for your symptoms and build a realistic plan — nutrition, targeted supplementation, and supportive therapies designed around your life.

Ongoing care & support

We walk with you as your plan unfolds, adjusting as your body responds. Our goal is lasting results, not a lifetime of medication.

How do I get started?

Call us at (813) 254-5200 or book online. We'll get you scheduled for your $79 New Patient Special — usually within the same week.

Which intake form should I fill out?

Choose the form that matches you — the women's intake includes reproductive-health questions, and the men's includes prostate and testosterone questions. Both cover the full functional-health assessment. Complete it before your visit if you can.

Do you take insurance?

We're a self-pay practice and don't bill insurance. That lets us spend real time with you and build a plan around your whole health. Many patients use HSA/FSA funds.

What should I bring to my first visit?

Any recent lab work or records you have, a list of current medications and supplements, and your completed intake form. Most importantly, bring your questions.
